This form is to be filled out by Delegation DCP's or their designee. Names should be listed as you would like them to appear on the registrants' nametag.

Late Fees Notice:
Any delegations submitting their registration after the registration deadline will be assessed a late fee of $5 per member. Any delegation that does not submit their payment or proof of payment before the close of registration on the first day of the event shall be assessed a late fee of $5 per member.
