Instructions:
This form is to be filled out by Delegation DCP's or their designee. Names should be listed as you would like them to appear on the registrants' nametag. All members of the organization are to be registered, including the Council of State. If a member is graduating please choose yes under the heading graduating senior.

Late Fees Notice:
Any delegations submitting their registration after the registration deadline will be assessed a late fee of $5 per member. Any delegation that does not submit their payment or proof of payment before the close of registration on the first day of the event shall be assessed a late fee of $5 per member.

Notes: Please choose a preference of eithier House or Senate and which committee you would prefer to serve on. (Reminders: If you registered for the Fall 2008 Interim Assembly or Spring 2009 LTI/Interim Assembly you should choose the same committees as your preferences for those events, as committee members serve on the same committee for the entire year. Additionally, the Speaker of the Assembly makes all decisions regarding Committee and Chamber Assignments.)

Your designated member should be the same member who served on the Rules Committee at the Fall 2008 Interim Assembly or Spring 2009 LTI/Interim Assembly if they are registering for this event.
